The following are some commonly used contracts, forms and agreements that have been approved for use by authorized Illinois Tech administrators, faculty members and staff.
Authorized Negotiate and Sign

These documents are general, and anyone using them should call the Office of General Counsel to discuss any modifications that may be necessary in the particular situation.
Agreement for Consulting Services
(non-sponsored research related)
For use when retaining a third party to render services to Illinois Tech and the services to be rendered do not relate to a project funded pursuant to an award or grant. Requires issuance of a purchase order number from Illinois Tech's Purchasing Department prior to execution. Contact IIT Purchasing for assistance.
Agreement for Consulting Services

For use when retaining a third party to render services to Illinois Tech and the services to be rendered will be funded pursuant to an award or grant. Must be processed through and issued by the Office of Sponsored Research and Programs (OSRP). Contact OSRP for assistance.
Assumption of Risk and Release
For use only when non-minors involved in educational programs at or sponsored by Illinois Tech.
Consent to Participate, and Assumption of Risk and Release
For use when minors are involved in educational programs at or sponsored by Illinois Tech.
Assumption of Risk and Release
For use with Illinois Tech students, faculty or staff engaging in non-work related physical activities.
Mutual Non-disclosure and Confidentiality Agreement
Non-Disclosure agreements must be executed by the Associate Vice Provost for Research or the Director of the Office of Sponsored Research and Programs.
